Auction Donation Form

We are thrilled to receive any donated items for the purposes of raising money at our Swing for the ARTS Silent Auction. The sky is the limit on what could be donated- gift baskets with samples, gift cards, experiences, memberships, subscriptions!

The ARTS is a tax-exempt organization status under section 501(c)(3) of the Internal Revenue Code with the IRS. Contributions to organizations with pending 501(c)(3) status may be tax deductible. Please consult your tax advisor to determine deductibility of this contribution. In case you may need it here is our EIN -87-2749911.

When filling out the form below please provide to the best of your ability complete and accurate information as so we can send an acknowledgment for your tax records.
